[applause]
00:00:12
usually when I think about the
00:00:14
human immunodeficiency virus, clearly
00:00:16
everyone recognizes this song, this song
00:00:20
was written by Queen at a time
00:00:24
when the end of the life of Freddie
00:00:26
Mercury, who had been struggling for a long time, was approaching
00:00:29
HIV infection and in the end turned out to be a
00:00:32
loser in this fight.
00:00:34
Almost 30 years have passed since Freddie’s death and
00:00:38
medical science has leapt far forward
00:00:40
and in this video I will
00:00:44
tell you as clearly as possible about the human immunodeficiency virus.
00:00:46
Today’s episode you will find out how HIV
00:00:50
appeared,
00:00:51
what is the difference between HIV and AIDS
00:00:54
can you get infected through a kiss and what is
00:00:57
the likelihood of catching it during
00:00:59
sexual intercourse
00:01:00
is there a cure for HIV and are there
00:01:03
people who have recovered from HIV is it scary to
00:01:06
operate on someone infected with HIV and is it possible to
00:01:09
work as a surgeon if you catch
00:01:11
the infection is it possible to have a scourge to give birth to a
00:01:14
healthy child, see today's episode
00:01:18
let's go
00:01:22
[music]
00:01:25
at the beginning of this episode I will show that, in
00:01:29
principle, anyone can do a test for HIV
00:01:32
and they can buy it anonymously, as if they had bought a regular test
00:01:35
at a pharmacy and I unfortunately found a test
00:01:39
that is done on the blood, let me
00:01:41
have to, but after all, mine little finger
00:01:44
or ring finger, but in general there are
00:01:48
tests when you can take it, rub the mucous membrane
00:01:53
from the gums and after 15 minutes
00:01:56
you get the result, it’s completely harmless,
00:02:00
but it’s 99 percent accurate, now let’s
00:02:03
see, we have a scarifier, I’ll tell you
00:02:07
everything, that is, a small little one, how many times will
00:02:12
I pierce
00:02:13
myself? the skin and here we have test strips and
00:02:23
so now we pierce my skin, gently
00:02:33
process it, hide the napkin and
00:02:38
make an injection
00:02:44
q&a, a
00:02:49
drop fell and you need to add two
00:02:54
drops of reagent for dilution, I think that’s
00:02:57
what we add 2 corps liked
00:03:04
[music]
00:03:05
this is what my test looks like and in 15
00:03:10
minutes we will get the result and you will find out
00:03:12
at the end of the episode Yuri Dut made a
00:03:15
great documentary about HIV and I
00:03:18
recommend watching it if you haven’t already
00:03:20
done it and she did it because the
00:03:24
HIV pandemic is of great importance for
00:03:27
our country and the world as a whole and this is
00:03:29
undoubtedly a pandemic, with kingdoms higher
00:03:32
on earth long before the coronavirus and
00:03:36
not yet going to give up their positions, which is why
00:03:38
everyone should know what
00:03:41
HIV is and how to live in a world where
00:03:44
every day we come into contact with HIV
00:03:46
positive people, we have the risk of becoming infected with
00:03:49
this dangerous virus
00:03:51
why I I say so confidently that
00:03:53
every day we meet HIV-
00:03:56
positive people throughout the
00:03:58
existence of HIV infection,
00:04:00
it has affected more than 60 million people in the world,
00:04:05
and
00:04:08
of these, more than twenty-five
00:04:13
million have already died; this population of the entire
00:04:19
Australia, so you know, so many people
00:04:23
have died because of this There are
00:04:27
now about
00:04:35
one and a half million people infected with the dangerous HIV virus in Russia, which means that
00:04:42
HIV infections have become infected and are now
00:04:45
infected,
00:04:47
more than one percent of the population of Russia is carriers,
00:04:51
that is, every 100 people it
00:04:55
meets are infected with HIV and this is not
00:04:58
necessarily a person who leads an
00:05:00
unhealthy lifestyle,
00:05:02
homosexuals or a prostitute, God forbid,
00:05:06
it could be any person, a
00:05:10
normal person, our citizen,
00:05:13
and the number of infected people in Russia is
00:05:17
growing steadily, and in 2017 this
00:05:23
figure was simply catastrophic;
00:05:25
in one year,
00:05:28
HIV
00:05:31
was infected; more accurately, there were more than a hundred thousand people identified as new cases of HIV infection, but this is
00:05:35
simply terrible indicator and this year
00:05:38
we were simply in the top three worst countries
00:05:41
along with two African countries.
00:05:43
What does this mean on the one hand, most
00:05:46
likely it means hushing up the problem
00:05:48
in past years, on the other hand this is a
00:05:51
good detection of patients now
00:05:54
thanks to the good and coordinated
00:05:56
work of
00:05:57
sleep centers that are actively checking and
00:06:02
they are testing the population, where did the
00:06:05
HIV epidemic begin, now about everything in order,
00:06:07
following the main hypothesis about HIV, there are
00:06:10
several types of monkey immunodeficiency virus,
00:06:13
and at some point
00:06:16
two types of this virus got into one monkey at once,
00:06:19
and through no biological
00:06:22
magic, a virus familiar to us all appeared
00:06:25
human immunodeficiency, but how did it get
00:06:28
from a monkey to a person,
00:06:30
of course, you can fantasize a lot and
00:06:32
flip through the pages of the hub plan, but I don’t recommend
00:06:35
doing this, most likely a person became
00:06:38
infected with it while cutting up a
00:06:40
chimpanzee carcass in Africa, where monkeys are the
00:06:42
object of hunting, and just when cutting the
00:06:45
carcass, a freshly killed chimpanzee virus
00:06:48
penetrated into humans, then through migration,
00:06:51
Africans came to the Western Hemisphere and
00:06:54
to a country that was a popular
00:06:56
destination for tourism, including sex
00:06:59
tourism, Haiti from there and
00:07:02
spread to the United States
00:07:03
where, by the way, Gaitan Arc was considered patient zero for a long time,
00:07:06
this is a flight attendant
00:07:09
from Canada who had a fairly
00:07:11
active homosexual sexual relationship life,
00:07:14
but who exactly was this patient zero is
00:07:17
still unknown, it is known that the
00:07:20
virus flew from Haiti to the USA somewhere in
00:07:23
1971 and all this coincided with the
00:07:26
sexual revolution, which arrived at just the
00:07:28
right time for this virus;
00:07:31
subsequently, in the early 80s, doctors
00:07:34
noticed more frequent cases of pneumonia,
00:07:37
pneumonia and Kaposi's sarcoma in
00:07:40
homosexual men, and only later did they
00:07:43
find an infectious agent
00:07:46
that causes a deficiency of immunity for the
00:07:48
discovery of HIV by French scientists already in the 21st
00:07:51
century, we gave the Nobel Prize, but at the
00:07:54
dawn of the epidemic,
00:07:58
so many labels were hung on homosexual men as always and
00:08:01
this time with to the species when the
00:08:04
Chinese were accused then all the dogs were unleashed on the
00:08:07
LGBT community by the way about this film the
00:08:10
Dallas Buyers Club for the sake of
00:08:12
which Matthew McConaughey lost so
00:08:14
much weight to look exactly like an
00:08:17
AIDS patient, that is, the last stage of
00:08:20
HIV infection, this film is just about prejudices and
00:08:23
discrimination, and the less we
00:08:26
know about the disease, the more
00:08:28
prejudices we have, so that’s why I
00:08:30
’ll tell you further, the HIV virus is an RNA
00:08:34
containing a virus, let’s draw it
00:08:36
[music]
00:08:40
it looks quite friendly,
00:08:42
but not everything is so simple, it damages those
00:08:47
cells, these are immune cells that are
00:08:52
part of have a special
00:08:54
cd4 protein, the so-called cd4 cells, these are
00:09:03
mainly t-lymphocytes, mainly
00:09:05
t-helpers, these are also monocytes,
00:09:13
macrophages and microglial cells, even only
00:09:19
those cells that are even in the
00:09:21
brain, but in addition to cd 4 protein,
00:09:25
our virus also needs a certain
00:09:29
protein to join it called
00:09:36
the number of different letters, for example cx4 or
00:09:43
others from the name, and in order to
00:09:47
penetrate the cell and give the cell
00:09:52
its own genetic material and build
00:09:58
its own DNA, this is my market virus needs to
00:10:04
adapt to these two
00:10:06
proteins and then it will penetrate the cell,
00:10:10
but if in this there is a mutation in the protein,
00:10:17
then this virus simply will not
00:10:21
penetrate the cell, and this means that a
00:10:24
certain percentage of people who have
00:10:27
certain mutations in this protein on the
00:10:30
surface of immune cells, they
00:10:33
simply cannot get HIV
00:10:36
infection if there are mutations in the proteins about
00:10:39
which I told you that the virus simply
00:10:42
cannot penetrate a cell, just like a
00:10:44
person cannot enter an apartment of the century if he does not have
00:10:47
keys, so some percentage of people are
00:10:49
generally susceptible to HIV. This is
00:10:52
based on the principle by which three
00:10:54
people have already been completely cured of HIV; the
00:10:56
fact is that they were sick also leukemia,
00:11:00
that is, blood cancer, and they were transplanted with
00:11:03
bone marrow from donors who had the
00:11:06
same mutations. As a result, the virus
00:11:08
disappeared from the bodies of these patients
00:11:11
and is not detected in the blood even in the
00:11:13
absence of antiretroviral therapy,
00:11:15
touring therapy against HIV, but how does
00:11:18
HIV infection itself proceed?
00:11:20
it shows the course of HIV infection in the form of a graph
00:11:24
where at the beginning there are practically no viral
00:11:29
units, a harmful virus that comes out
00:11:34
like the sun, but there are a lot of CD4 cells in the body, the
00:11:38
number of viruses begins
00:11:43
to increase, they grow in our blood,
00:11:46
thereby reducing the number of CD4
00:11:52
cells affecting and in connection with this the
00:11:56
acute phase begins
00:11:58
infections don and and the beginning we have a
00:12:02
period when the number of viruses does not
00:12:05
make itself felt at all; this incubation
00:12:08
period can last up to six months to
00:12:10
six months a person may not have
00:12:14
any symptoms at all,
00:12:15
but when the acute phase begins, people
00:12:18
sometimes describe it as the worst
00:12:21
mushroom in their life because this is a
00:12:27
flu-like course of the acute phase,
00:12:29
now it can probably be compared with the
00:12:31
corona virus, but yes, this is the most severe
00:12:35
fungus in their life, this is how the acute phase of
00:12:39
the disease passes, but as soon as the body
00:12:42
begins to slowly cope with
00:12:45
the infection on its own, the number of
00:12:48
antibodies increases and the CD4 cells are
00:12:51
slightly restored to a
00:12:54
certain level, the number of
00:12:57
viruses
00:12:59
decreases, but the body simply
00:13:03
cannot cope with the infection, we
00:13:05
know this if the infection is not treated in any way, then
00:13:08
it will inevitably lead to death and then a
00:13:12
latent period follows, which sometimes
00:13:15
may not give any signs
00:13:19
about itself, a person can also not
00:13:21
to notice it, but nevertheless, the number of CD4
00:13:23
cells will inevitably fall and
00:13:27
the number of viruses and the viral load will
00:13:30
grow as soon as the number of T-lymphocytes
00:13:34
drops to less than 200 units per
00:13:44
microliter, then the phase
00:13:51
known to everyone as AIDS, the last
00:13:55
terminal stage of HIV infection,
00:13:58
acquired syndrome, begins immunodeficiency here
00:14:02
the number of CD4 cells completely drops the
00:14:05
number of viruses increases since the
00:14:08
immune response cannot occur
00:14:11
practically to the most harmless
00:14:15
infections for a healthy person,
00:14:18
such as Tsesna pneumonia and pneumonia
00:14:21
or cancer
00:14:23
Kaposi's sarcoma, it is during this period that
00:14:27
these opportunistic ones develop
00:14:30
so-called infections because of which a
00:14:33
person dies,
00:14:35
but it is now known that even if
00:14:39
treatment is started at the stage of AIDS, it is still
00:14:42
possible to reverse the
00:14:44
course of the disease and save the person; the
00:14:47
transmission routes are quite simple,
00:14:51
usually in contact with a damaged
00:14:53
surface of the body or in
00:14:57
contact with a biological
00:14:59
liquid with damaged or, very
00:15:02
rarely, undamaged mucous membrane,
00:15:05
in other words, and
00:15:06
if there are no microcracks, then you may not
00:15:09
get infected at all, but if the
00:15:11
mucous membranes are traumatized during sexual intercourse,
00:15:13
for example, in this case anal
00:15:17
contact is more dangerous and leads to
00:15:19
traumatization, and it is the
00:15:22
sexual tract that gets infected first
00:15:26
place among the path and battles in connection with and in
00:15:28
connection with the intimate mucous membrane of the rectum,
00:15:30
homosexually unprotected contacts are
00:15:33
most dangerous, infection can occur
00:15:36
from mother to fetus or child
00:15:40
during feeding and vice versa, and of course
00:15:42
health workers, beauty workers
00:15:45
have a risk since working with
00:15:47
biological fluids, for example, I
00:15:50
always I put on these special
00:15:52
chain mail gloves so as not to get infected,
00:15:54
but can a doctor work as a
00:15:58
surgeon infected with HIV? Firstly, it is almost impossible to transfer HIV from a doctor to a
00:16:01
patient for surgery,
00:16:03
and secondly, according to the labor
00:16:06
code, a HIV-positive person in the
00:16:09
workplace or anywhere does
00:16:12
not pose a danger to others
00:16:15
and discrimination on the basis of the
00:16:17
disease, a complete violation of rights and
00:16:20
freedoms, it is necessary to understand that the virus is not transmitted through everyday contacts and
00:16:23
contacts, you can shake
00:16:26
hands and even kiss, provided that you
00:16:28
receive therapy, the risk of transmitting the virus
00:16:31
even through sexual contact tends to
00:16:33
zero,
00:16:34
and an infected pregnant woman can give birth to a healthy
00:16:37
child. there are many
00:16:39
HIV dissidents about them you need to know one thing
00:16:43
because of their activities infected mothers
00:16:46
gave birth to many HIV-positive children although
00:16:49
they could have avoided this
00:16:50
ruining themselves they ruin the lives of others
00:16:53
this is all you need to know about them there are
00:16:56
pages of history that you want to
00:16:58
completely forget about, for example outbreaks carried away in
00:17:00
hospitals of the USSR where
00:17:03
reusable syringes were also used, they were sometimes
00:17:06
carried properly and were implemented in general, and in general it was
00:17:08
considered dangerous only for residents of
00:17:10
capitalist countries,
00:17:12
homeless people, prostitutes and homosexuals, this
00:17:16
turned out not to be the case and the outbreak occurred in
00:17:20
the leaf and from there spread to
00:17:23
other cities, as in my favorite in
00:17:25
Volgograd, at least 35 children were infected
00:17:28
and now hardly any of them are
00:17:31
alive, this is completely sad, what is the
00:17:34
percentage of possible infection through
00:17:39
different routes of transmission during sexual intercourse,
00:17:48
this percentage is less, usually less than
00:17:52
one percent, even with anal
00:17:57
intercourse, the percentage is not so high then
00:18:00
there is most likely only 100 times an
00:18:04
infection can come, but as we know,
00:18:06
of course, basically it is 50 to 50 and
00:18:09
therefore no one knows for sure about this during
00:18:12
pregnancy, the
00:18:17
percentage of infection not by a pregnant
00:18:21
woman, the percentage of infecting her child is
00:18:25
about 30 percent, but with parenteral
00:18:33
routes, that is for example, with a
00:18:36
blood transfusion, this percentage is already more than 90,
00:18:47
in this case it is almost 100%
00:18:52
accurate and you can become infected, so the blood
00:18:57
that is taken from donors undergoes
00:19:00
quarantine, then it is possible to detect
00:19:03
HIV infection in a patient who has become a
00:19:05
Don, but if we take into account that the patient is a
00:19:12
carrier of HIV infection receives
00:19:15
antiretroviral therapy, follows everything correctly
00:19:18
and takes pills, then the percentage of
00:19:22
infection tends to 0 percent, that
00:19:31
is, now we understand that the main
00:19:33
prevention is correct and
00:19:37
during the detection of HIV infection
00:19:40
and of course
00:19:43
therapy, which
00:19:46
absolutely all patients should receive, absolutely all
00:19:49
carriers of HIV infection, regardless of
00:19:53
status, unfortunately in our country
00:19:57
this is a problem and not everyone receives
00:20:00
antiretroviral therapy,
00:20:01
so you need to follow protective measures
00:20:06
such as protected sex with a
00:20:10
condom
00:20:11
and others, but of course you are on us, this is
00:20:14
antiretroviral therapy and so
00:20:17
20 minutes have passed and my test
00:20:20
shows one strip not this doesn’t
00:20:23
mean that I’m not pregnant, although I
00:20:25
probably still won’t be able to get pregnant,
00:20:27
but it means that I’m HIV negative,
00:20:31
it means I don’t have HIV infection in my body
00:20:34
at the moment, but you need to
00:20:37
remember if, let’s say, I became infected
00:20:39
just yesterday or the day before yesterday or even
00:20:41
weeks ago or even six months,
00:20:45
sometimes such a period of sulfur is negative,
00:20:48
sometimes during this period when you are already
00:20:51
charged with the infection already in you, but the test may
00:20:55
still show a
00:20:57
negative result, in addition to this
00:21:00
test, and of course I take it, I take it once a
00:21:03
year, like other people who work in
00:21:07
medicine, for example but I did this
00:21:09
to show how simply and
00:21:11
almost anonymously you can take an
00:21:15
HIV test, and therefore if you have never
00:21:17
taken an HIV test before,
00:21:18
I recommend that you still do it and
00:21:21
if you get a positive result, then
00:21:24
don’t worry about just contacting the
00:21:26
center AIDS where they will tell you everything they will give you
00:21:28
in order how to proceed next
00:21:31
because now people don’t die from HIV what we
00:21:33
can do to prevent the
00:21:35
spread of HIV infection
00:21:37
firstly distribute this video among
00:21:39
your friends so it spreads
00:21:41
secondly get tested which
00:21:43
can be done anonymously and persuade
00:21:45
your partner
00:21:47
if HIV is detected, receive therapy and
00:21:51
provide all those infected with these drugs at the expense of the
00:21:53
state,
00:21:55
antiretroviral therapy is usually
00:21:58
several drugs at once, usually 3,
00:22:00
which stop the development of the virus at
00:22:03
different stages and prevent mutations,
00:22:06
including in it only in
00:22:08
small quantities and strains of the
00:22:10
virus, antiretroviral therapy is
00:22:12
not very effective in most cases
00:22:15
it is effective, in other cases it
00:22:16
helps stop illnesses and lead a
00:22:19
normal life and not be
00:22:22
dangerous to others at all; we also need to
00:22:25
ban
00:22:26
groups of HIV dissidents, that’s for sure, and
00:22:30
provide them with condoms for absolutely
00:22:33
everyone; first of all, schoolchildren and
00:22:34
students; provide disposable
00:22:37
syringes for injecting drug addicts;
00:22:40
yes this is By the way, this is not a joke, this is one of the
00:22:43
principles of the World Health Organization,
00:22:46
because it is unlikely that it
00:22:50
will work at all to prevent injection injections from taking the next dose, but to protect you
00:22:52
from infections, and in the end, you
00:22:56
only need to know two things: you should not be afraid of people
00:22:58
infected with HIV, but you should know about it and
00:23:02
take care of yourself and your loved ones,
00:23:05
the fight was a moment, that's all for now
